DCS Athletic Rules and Policy The are 10 simple rules that have been approved by Mr. Allegrezza for all Athletes at Deer Creek School to start following immediately. Medical Release Form This form is in the event you as the parent/gaurdian can't be contacted and medical treatment is needed during practice or a contest.  This just gives permission for an official of Deer Creek School to request the treatment. Consent to Participate Form This form is proof that you have allowed your son/daughter to participate in athletic activity at Deer Creek School.  Physical Form (OFFICIAL) This is the physical form that we MUST have on file BEFORE your son/daughter can participate, practice or play ANY athletic event for Deer Creek School teams or squads. 2008 Summer Conditioning Program Calendar Summer Conditioning information and dates.  Two lifting sessions 7:00 AM and 9:00 AM for all athletes enrolled at Deer Creek.
